Charitable Trust Registration Form.
This form asks whether or not your nonprofit will solicit funds from the general public or conduct fundraising events; if so, there may be additional provisions in state law that pertain to fundraising by nonprofit organizations. Check the Ohio Attorney General's website for more information http://www.ag.state.oh.us/
Charitable Solicitation Registration Statement. Ohio requires (annual) registration by some nonprofit organizations before any charitable solicitation can be conducted (based on the amount of funds generated). The registration may include information about professional fundraisers that are contracted to the nonprofit organization such as fees and/or salaries paid to the professional. Visit the Ohio Attorney General's website at http://www.ag.state.oh.us/
Statement of Continued Existence. Ohio requires nonprofit corporations to file a form that documents their continued existence every five years. Generally there is a fine applied for late filings, and failure to file may also result in the organization losing it’s status as a nonprofit corporation so that it must reapply and pay additional fees. This can also interrupt the tax-exempt status of the corporation because IRS tax-exempt status may be based on nonprofit incorporation in that state. The "reminder" notice is sent to the Statutory Agent, so be sure that address is kept updated with the Secretary of State's office.
